Hale Kupuna Heritage Home is an 84 bed skilled nursing and intermediate long term care center in Omao, on the island of Kauai. Nestled in a residential neighborhood away from the hustle and bustle of the city, residents enjoy the natural, lush, green surroundings, while benefitting from the services we offer.
Skilled & Intermediate (Long Term) Nursing Care
Registered and licensed nurses and certified nursing assistants provide 24 hour care. Services include medication management, food and nutritional evaluation, and specialized dietary support. Each resident is assessed and a plan of care is designed by an interdisciplinary team. This team includes the medical director, nurses, social worker(s), rehabilitation therapist(s), activities coordinator and dietitian. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments.
Our home encourages residents to participate in many social and cultural activities—individuals, small group and special events. Family members are welcome to join anytime. Residents, their family members and staff become well acquainted. There is a strong feeling of “home” and family here.
Other specialized services include advanced wound care, diabetes support, IV therapy, enteral nutrition, and pain management.
We offer physical, occupational, and speech therapy. In addition, our Restorative Nursing Program helps to maintain residents at their highest functioning level.
Families can benefit from a needed break from their caregiver duties by taking advantage of our short term stay options. Those who come to us for respite care may receive help with their medications, assistance with Activities of Daily Living (ADL’s- eating, bathing, dressing, grooming, toileting) and personalized care planning.
We work collaboratively with hospice providers to provide a nurturing and supportive environment so that residents may spend their final days in comfort.
