We strive to provide clinical excellence and genuine care through our multidisciplinary team approach.
Skilled Nursing & Specialty Care
Legacy Hilo provides assistance with the activities of daily living, including personal hygiene, dressing, eating, walking, toileting, & transferring from one position to another. Planned events provide residents with an opportunity to enjoy the companionship of others in a warm & caring family-oriented setting. Registered and licensed nurses and certified nursing assistants provide 24-hour care. Services include medication management, food and nutritional evaluation, and specialized dietary support. Each resident is assessed and an individualized plan of care is designed by an interdisciplinary team. This team includes the medical director, in-house physician, licensed nurses, social worker(s), rehabilitation therapist(s), activities coordinator and dietitian. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments.
